HPV Viral Load-Based Triage in Cervical Cancer Screening: China’s Standardized Quantitative Approach Gains International Attention

Human papillomavirus (HPV) DNA testing offers high sensitivity for cervical cancer screening, but its limited specificity can result in unnecessary follow-up procedures. Standardized HPV viral load quantification is emerging as a promising approach to improve risk stratification and triage among HPV-positive individuals. This article reviews the recently published Chinese Expert Consensus on the Use of Human Papillomavirus Viral Load for Cervical Cancer Screening and Triage alongside an international review in Expert Review of Molecular Diagnostics. Both publications emphasize quantitative real-time PCR methods that normalize HPV viral load to sample cellularity using a human single-copy gene as an internal reference. This approach helps reduce sampling bias and enables more reproducible comparisons across specimens. The article also discusses the BioPerfectus BMRT HPV Genotyping Real-Time PCR workflow, which combines extended HR-HPV genotyping with TOP3-based cell quantification, as well as the use of ROC curve analysis to establish clinically relevant viral load cutoffs. Although further multicenter validation and international standardization are required, HPV viral load combined with genotyping may support more precise cervical cancer screening, reduce unnecessary colposcopy referrals, and enable risk-based clinical management.
